Researchers at Universiti Malaysia Kelantan have completed a three-year investigation demonstrating that protein found in the saliva of local buffalo leeches possesses significant potential as an anticoagulant, opening fresh possibilities for Malaysia's biotechnology and pharmaceutical industries. The discovery could unlock commercial applications for a species that has been largely overlooked in contemporary medical development, despite Asia's reputation as a global source of premium therapeutic leeches.

Dr Zulhisyam Abdul Kari, a senior lecturer at UMK's Faculty of Agro-Based Industry, explained that the extracted protein contains hirudin, a naturally occurring compound with anticoagulant properties comparable to heparin, the synthetic substance routinely administered in hospitals to prevent blood clotting. Controlled laboratory testing confirmed that the leech protein extract effectively delays coagulation to degrees equivalent to conventional anticoagulants used in clinical settings today. This breakthrough carries particular significance for Malaysia, as it demonstrates that indigenous biological resources can compete with imported pharmaceutical alternatives.

The research team selected the buffalo leech (Hirudinaria manillensis) specifically because these creatures produce substantially greater volumes of saliva with superior protein quality compared to terrestrial leech species. Buffalo leeches thrive in aquatic environments and have evolved to produce more potent salivary secretions, making them inherently more suitable for protein extraction and pharmaceutical applications. This biological advantage positions buffalo leeches as a naturally optimised source material for anticoagulant development.

Leech-based medicine carries a distinguished history spanning centuries across multiple cultures. Traditional applications have addressed conditions ranging from gout and acne to chronic wounds in diabetic patients, while modern commercial leech therapy operates under rigorous clinical protocols where each leech is used exclusively for a single patient to eliminate cross-infection risks. Leeches cultivated across Asia command premium prices in international markets because of their superior therapeutic quality. Germany and other developed economies maintain substantial imports from the region, with Southeast Asian leeches frequently utilised in pre-operative treatments preceding cardiac surgery to ensure optimal blood circulation during procedures.

Thailand has imported Malaysian leeches to supply its own expanding medical sector, reflecting the regional demand for high-quality specimens. However, Malaysia's leech farming sector has contracted dramatically in recent decades despite earlier expansion through contract farming initiatives. Dr Zulhisyam attributed this decline partly to fraud scandals that eroded investor confidence and disrupted supply chains. The collapse of trust within the industry prompted local stakeholders to withdraw capital and shift focus toward alternative agricultural ventures. This contraction represents a missed economic opportunity, as neighbouring countries capitalised on Malaysian breeding expertise to establish their own competitive operations.

Environmental pressures compound the farming industry's struggles. Natural leech populations in traditional habitats such as paddy fields have deteriorated significantly due to agricultural intensification. The widespread application of chemical pesticides and synthetic fertilisers has degraded wetland ecosystems where wild leeches naturally breed, creating population bottlenecks that threaten species sustainability. Without adequate wild populations to supplement farmed stocks, the industry faces acute resource limitations that undermine expansion prospects.

Responsibility for leech management and conservation transferred entirely to the Department of Fisheries Malaysia in 2011, moving jurisdiction away from the Department of Wildlife and National Parks. This institutional shift reflected growing recognition of leeches' value as aquatic biological resources rather than purely wildlife specimens. The change in oversight body signals an implicit commitment to treating leeches as economically significant assets worthy of sectoral management frameworks.

Dr Zulhisyam emphasised that unlocking commercial potential requires substantially increased research funding, particularly for advanced molecular biology investigations capable of characterising the precise mechanisms through which leech proteins function as anticoagulants. Without dedicated investment in this foundational science, Malaysia risks remaining dependent on imported anticoagulant drugs despite possessing superior source organisms within its own borders. Insufficient funding leaves the research incomplete and commercialisation prospects unrealised.

The researcher urged society to reconceptualise leeches as valuable natural assets meriting preservation for future scientific advancement rather than viewing them as parasitic nuisances. Each organism contributes essential functions to broader ecological systems, and leeches' roles in aquatic food webs and nutrient cycling remain incompletely understood. Protecting leech populations strengthens ecosystem resilience while maintaining populations available for legitimate pharmaceutical extraction and biotechnology applications.

The potential commercialisation of leech-derived anticoagulants represents a distinctive opportunity for Malaysia to establish competitive advantage in high-value biotechnology markets. Asian leeches already command premium valuations in international pharmaceutical procurement, yet Malaysia has not fully leveraged this position through structured research and industry development. Converting academic discoveries into commercial products requires coordination between universities, government agencies, and private investment—elements currently misaligned following decades of industry contraction.

Successful development could generate export revenue and establish Malaysia as a recognised source of naturally derived pharmaceutical compounds, enhancing the nation's biotechnology profile globally. However, realising this potential demands immediate action: increased research funding, restoration of farmer confidence through transparent regulatory frameworks, and environmental protection for wild leech populations. Without comprehensive intervention, the opportunity may migrate to competitors in Thailand, Vietnam, or other regional players possessing greater commitment to biotechnology development infrastructure and capital allocation.
